Payment for services provided is ultimately the patient’s responsibility. As a courtesy, our office will submit claims to your insurance company on your behalf.
Our practice participates with many commercial insurance plans, as well as Medicare and Medicaid plans. Because coverage varies by plan, we encourage you to contact our office with any questions about participation. You should also verify your benefits directly with your insurance carrier, including your eligibility, copayments, coinsurance, deductibles, and any coverage limitations before receiving services.
Any portion of the charges not covered by your insurance plan is the patient’s responsibility. Payment will be collected by credit or debit card and may be made securely online or by phone.
If you are an uninsured patient or we are not in-network with your insurance, the cost for initial visit is $365 and for each follow up visit is $200.
